How to get your VaxCertPH: A Step-by-Step Guide

We’ve seen Vaccine Cards in a wide variety of shapes and sizes — some LGUs have created documents that fit your wallet, while some created certificates that require envelopes to carry around. The good news is, your vaccination certificate can now be digitized by obtaining your VaxCertPH COVID-19 Digital Vaccination Certificate from the Department of Health (DOH).

What is VaxCertPH?

According to the DOH, VaxCertPH is the official Digital Vaccination Certificate (DVC) for Filipinos and non-Filipinos vaccinated in the Philippines that may be used for international and domestic travel. It is free of charge, and complies with the WHO Digital Documentation of COVID-19 Certificates (DDCC) guidelines.

How do I obtain my DVC from VaxCertPH?

Vaccination data from your LGU is uploaded into the VaxCertPH central database at least 48 hours after your 2nd dose — after the waiting period, you may then request for your digital certificate from a physical VaxCertPH booth or through the VaxCertPH website at https://vaxcert.doh.gov.ph/.

Generating your DVC online

1. Visit https://vaxcert.doh.gov.ph

2. Click “Continue” to request for a Vaccination Certificate

3. Review and Accept the Data Privacy Statement

4. Answer the Personal Information Forms

5. Complete the Vaccination Details Form

6. Verify the Information (Click "Continue") and Complete the Captcha Prompt

7. Wait for the website to look up your Vaccine Data

8. Receive & Download the Vaccination Certificate

Verification of DVCs

Digital Vaccination Certificates provided through VaxCertPH can be verified by scanning the QR code provided on the document, simply visit the website and click on “Scan QR Code”. Do note that this will require you to have an attached camera on your device (e.g., Webcam or Mobile Camera).

Updating your VaxCertPH Record

For booster doses, you may request to update your records from the same website. Simply click on “Update Record” to begin the process of updating. You will need to upload a copy of your vaccination card and one (1) government-issued ID, together with your email address and/or contact number in order to request for an update request from your LGU.

For further inquiries, you may reach the DOH VaxCertPH team directly through the following communication channels

  • Customer Hotline (Telephone): 88-7614-88
